Who Can Make a Request? Requests for information are available to NDIS Participants; child representatives; nominees; third parties who have consent from a participant to access and share their information and prospective participants or representatives in making the access request under the NDIS Act.
How to make a Request. You can make a request online by email or over the phone. Requests by email are sent to: information.access@ndis.gov.au or by phone on: 1800 800 110
You will need to provide some information to identify and verify who you are. This includes: your full name; date of birth; address and NDIS reference number.
What Type of information can be requested? You can request information about your access to the NDIS or your NDIS Plan or plan review.
If you are requesting information about your NDIS plan or plan review, the NDIS can provide you with information regarding your approved plan; information collected at your planning meeting; assessments and other information collected in support of your plan or plan review; and reasons for your plan review decision.
